In the form of injections (mainly in acute conditions), a single dose is 20 mg, with good tolerance, the dose is increased within 3-4 days to 1 mg / kg; the duration of treatment is 10-14 days.
vinpocetine
Pregnancy, hypersensitivity to vinpocetine.
For parenteral use: severe coronary artery disease, severe arrhythmias.
pharmachologic effect
An agent that improves cerebral circulation and cerebral metabolism. Inhibits the activity of phosphodiesterase, which contributes to the accumulation of cAMP in tissues. It has a vasodilating effect mainly on the vessels of the brain, which is due to the direct myotropic antispasmodic effect. Systemic blood pressure decreases slightly. Improves blood supply and microcirculation in the tissues of the brain, reduces platelet aggregation, and helps to normalize the rheological properties of blood. Improves the tolerance of hypoxia by brain cells, facilitating the transport of oxygen to tissues due to a decrease in the affinity of erythrocytes to it, increasing the absorption and metabolism of glucose. Increases the content of catecholamines in the brain tissues.
Pharmacokinetics
When taken orally, it is rapidly absorbed from the gastrointestinal tract. Cmax in blood plasma is reached after approximately 1 hour. With parenteral administration, Vd is 5.3 l / kg. T1 / 2 is about 5 hours.
Side effect
From the side of the cardiovascular system: transient arterial hypertension, tachycardia; rarely - extrasystole, an increase in the time of excitation of the ventricles.
Application during pregnancy and lactation
Contraindicated in pregnancy. There are no data on the safety of use during lactation.
special instructions
Use with caution inside with arrhythmia, angina pectoris, with unstable blood pressure and decreased vascular tone. Parenterally, as a rule, they are used in acute cases until the clinical picture improves, then they switch to oral administration. For chronic diseases, vinpocetine is taken orally. In hemorrhagic cerebral stroke, parenteral administration of vinpocetine is allowed only after a decrease in acute events (usually 5-7 days).
Drug interactions
With simultaneous use with warfarin, a slight decrease in the anticoagulant effect of warfarin has been reported. With the simultaneous parenteral use of vinpocetine and heparin, an increase in the risk of bleeding is possible.
