Pharmacological action
Cyanocobalamin has a hematopoietic, stimulating cellular metabolism effect. Cyanocobalamin is involved in the transfer of methyl fragments, the formation of choline, methionine, creatine, nucleic acids, the maturation of red blood cells, contributes to the accumulation of compounds containing sulfhydryl groups in red blood cells.
Indications
Chronic anemia with cyanocobalamin deficiency (Addison-Birmer disease, alimentary macrocytic anemia), as well as in the complex treatment of iron deficiency, posthemorrhagic and aplastic anemia, anemia caused by toxic substances and.
Diseases of the nervous system: polyneuritis, radiculitis, causalgia and neuralgia (including trigeminal neuritis), amyotrophic lateral sclerosis, peripheral nerve injuries, funicular myelosis, cerebral palsy, Down's disease.
Skin diseases (psoriasis, photodermatosis, herpetiform dermatitis, atopic dermatitis).
Chronic hepatitis, cirrhosis, liver failure.
Acute radiation sickness.
For prophylactic purposes - when prescribing biguanides, paraaminosalicylic acid, ascorbic acid in high doses, pathology of the stomach and intestines with impaired absorption of cyanocobalamin (resection of a part of the stomach, small intestine, Crohn's disease, celiac disease, malabsorption syndrome, spru).
Contraindications
Hypersensitivity to the components of the drug, thromboembolism, erythremia, erythrocytosis, pregnancy (there are separate indications of the possible teratogenic effect of B vitamins in high doses), lactation, children under 3 years of age for this dosage form. Leber's disease, nicotine amblyopia (increases the risk of neurodegenerative damage to the optic nerve).
Angina of exertion FC III, benign and malignant neoplasms (except in cases accompanied by megaloblastic anemia and vitamin B12 deficiency), tendency to form blood clots.
Special instructions
Vitamin B12 deficiency must be confirmed by diagnostics before prescribing, because it can mask the lack of folic acid.
During treatment, it is necessary to monitor peripheral blood counts: on the 5-8th day of treatment, the number of reticulocytes and the concentration of Fe are determined. The number of red blood cells, Hb and the color index must be monitored for 1 month 1-2 times a week, and then 2-4 times a month. Remission is achieved by increasing the number of red blood cells to 4-4.5 million / μl, with the achievement of the normal size of red blood cells, the disappearance of aniso and poikilocytosis, normalization of the number of reticulocytes after a reticulocytic crisis. After achieving hematological remission, peripheral blood control is performed at least 1 time in 4-6 months.
Use caution in patients prone to thrombosis with angina pectoris (in lower doses, up to 0.1 mg per injection). Take for a long time with pernicious anemia, upcoming gastrointestinal operations.
When used in recommended doses in elderly people, there were no adverse reactions other than those listed above.
Composition of
1 ampoule (1 ml of solution) contains:
of the active substance - cyanocobalamin - 500 mcg
excipients - sodium chloride, water for injection.
Side effects
Allergic reactions, mental agitation, cardialgia, tachycardia, diarrhea, headache, dizziness.
When used in high doses - hypercoagulation, impaired purine metabolism.
Drug interaction
Pharmaceutically incompatible with ascorbic acid, salts of heavy metals (inactivation of cyanocobalamin), thiamine bromide, pyridoxine, riboflavin (because the cyanocobalamin ion is destroyed in the molecule of cyanocobalamin.
Aminoglycosides, salicylates, antiepileptic drugs, colchicine, K + drugs reduce absorption. Enhances the development of allergic reactions caused by thiamine.
Chloramphenicol reduces the hematopoietic response.
Can not be combined with drugs that increase blood coagulation.
Risk of developing allergic reactions with thiamine.
