DESCRIPTION

Semintra is a drug used to reduce proteinuria in cats with chronic kidney disease. In appearance, the drug is a transparent viscous liquid from colorless to yellowish.

COMPOSITION

The drug contains 10 mg of telmisartan as an active ingredient in 1 ml, and as auxiliary substances: benzalkonium chloride, hydroxyethyl cellulose, sodium hydroxide and hydrochloric acid (for adjusting the pH level), maltitol and purified water.

PHARMACOLOGICAL PROPERTIES

Pharmacotherapeutic group: angiotensin II antagonists.

Telmisartan is a specific angiotensin II receptor antagonist that causes a dose-dependent decrease in arterial blood pressure in mammals, including cats, and reduces proteinuria. In a clinical trial in cats with chronic kidney disease, a decrease in proteinuria was noticeable in the first 7 days of treatment.

Telmisartan displaces angiotensin II from its association with the AT] receptor. Telmisartan binds selectively to the ATi receptor and does not bind to other AT receptors. Stimulation of the ATi receptor is responsible for the pathological effects of angiotensin II in the kidneys and other organs associated with angiotensin II, such as vasoconstriction (narrowing of the blood vessels), sodium and water retention, increased aldosterone synthesis, and organ changes. At the same time, the effects associated with stimulation of the ATg receptor, such as vasoconstriction, natriuresis and inhibition of inappropriate cell growth, are not suppressed. The receptor binding persists for a long time due to the slow dissociation of telmisartan from its association with the AT] receptor. Telmisartan has no ATi receptor agonist activity,

When administered orally to cats, telmisartan is rapidly absorbed, reaching a maximum plasma concentration after 0.5-1 hour. Food consumption does not significantly affect the overall absorption of telmisartan. Telmisartan is a lipophilic compound that has membrane permeability, which facilitates natural distribution in tissues and organs. No peculiarities of the drug's action on animals of different sex were revealed.

No clinically significant accumulation of the drug in organs and tissues was observed during its prolonged use (within 21 days). The absolute bioavailability after oral administration was 33%. Telmisartan binds to plasma proteins (> 99.5%), mainly albumin and a-1-acid glycoprotein. The half-life averages 7.7 hours.

Telmisartan is metabolized in the liver to the pharmacologically inactive glucuronide (1-Oaciloglucuronide telmisartan). It is excreted from the body mainly with feces. The seminar, according to the degree of impact on the body, belongs to low-hazard substances (hazard class 5 according to GOST 32296-2013).

DOSAGE AND APPLICATION

Semintra is indicated for the reduction of proteinuria in cats with chronic kidney disease (CKD) as a monotherapy.

The drug is administered orally once a day to cats over 6 months of age. The drug is injected with a syringe-dispenser directly into the oral cavity. It is possible to use the preparation with a small amount of feed.

1. Press the bottle cap down, twist and open it. Insert the syringe-dispenser into the nozzle of the vial stopper by lightly pressing. Tilt the syringe bottle down. Pull the plunger to the graduation value on the syringe corresponding to the cat's weight in kilograms. Remove the syringe dispenser from the bottle.

2. Press down on the plunger to transfer the contents of the syringe directly into the cat's mouth or into a small amount of food.

Close the bottle tightly after taking the drug, rinse the syringe with water and let it dry.

The therapeutic dose and duration of the course of treatment, depending on the physical condition of the animal and the course of the disease, is determined by the attending veterinarian.

Semintra can be used in the complex therapy of chronic kidney disease with other drugs. The duration of the course of treatment is not limited.

SPECIAL INSTRUCTIONS

In case of an overdose of the drug, animals may experience belching, vomiting, diarrhea or loose stools, hypotension, and a decrease in the number of erythrocytes.

The peculiarities of the pharmacological action on the animal organism during the first application of the drug and upon its withdrawal were not revealed.

CONTRAINDICATIONS

The drug is not used in animals with hypersensitivity to the active and / or auxiliary substances.

The drug should be administered with caution in chronic heart failure, hypovolemia, hyponatremia, hyperkalemia. In these cases, periodic examinations of the veterinarian and monitoring of blood counts are necessary.

The drug is not used in pregnant and lactating cats, as well as animals younger than 6 months of age.

SIDE EFFECTS

When using the drug in accordance with this instruction, in some cases, animals may experience mild and / or intermittent belching, vomiting, diarrhea or loose stools, hypotension and a decrease in the number of red blood cells, an increase in the level of liver enzymes. In connection with the latter, it is necessary to monitor blood counts during the entire period of treatment.

In very rare cases, there is an increase in the level of liver enzymes, which returns to normal within a few days after stopping therapy.

In case of individual hypersensitivity of the animal to the drug and the appearance of allergic reactions, the drug is stopped and the animal is prescribed antihistamines and symptomatic agents.

SHELF LIFE AND STORAGE

The shelf life of the drug, subject to storage conditions, is 3 years from the date of production. The shelf life of the drug after opening the bottle is 6 months. The medicinal product must not be used after the expiration date.

The seminar is stored in sealed manufacturer's packaging separately from food and feed, protected from light, at a temperature of 10 °C to 25 °C.

The drug is produced in plastic bottles of 30 ml. The bottle is packed with instructions for use and a syringe-dispenser for oral administration in an individual cardboard box.
