Symptomatic treatment of nasal congestion in acute respiratory diseases with symptoms of rhinitis (runny nose), acute allergic rhinitis, hay fever, sinusitis, eustachitis, otitis media (to reduce swelling of the nasopharyngeal mucosa).
To facilitate the outflow of secretions in diseases of the paranasal sinuses.
Preparing the patient for diagnostic manipulations in the nasal passages.
Internasally.
Adults and children over 12 years old: 1 injection into each nasal passage 1-2 times a day. Should not be used more than twice a day. Before first use, spray the nasal spray with several taps until the drug is evenly distributed.
1. Do not cut off the spray tip.
2. Thoroughly clean your nose.
3. Remove the protective cap. Hold the bottle vertically, supporting the bottom with your thumb, the nozzle between your middle and forefinger.
4. Direct the tip into the nasal passage by tilting the bottle forward slightly.
5. Press the nebulizing device while nebulizing the drug and inhale through the nose at the same time.
6. Repeat for the other nasal passage.
7. After use, clean and dry the tip before closing it with the protective cap. In the recommended dose, without consulting a doctor, use for no more than 7 days. Do not exceed the recommended dosage.

Hypersensitivity to the components of the drug, transphenoidal hypophysectomy and surgical interventions on the membranes of the brain (in history), angle-closure glaucoma, pregnancy, atrophic rhinitis, age up to 12 years.
Carefully:
Increased intraocular pressure, arterial hypertension, hyperthyroidism, diabetes mellitus, pheochromocytoma, porphyria, prostatic hyperplasia, diseases of the cardiovascular system. It is recommended to be careful when treating patients receiving MAO inhibitors at this time or having received them within the previous 2 weeks, tri- and tetracyclic antidepressants, as well as other drugs that increase blood pressure. Pharmacodynamics
OtrivinЃ Express (oxymetazoline) is a sympathomimetic agent that acts on the alpha-adrenergic receptors of the nasal mucosa. It has a vasoconstrictor effect, reducing the swelling of the nasopharyngeal mucosa. When applied topically to the inflamed nasal mucosa, it reduces swelling and nasal discharge. Restores nasal breathing, eliminates swelling of the nasal mucosa, helps to restore aeration of the paranasal sinuses, middle ear cavity, which reduces the likelihood of bacterial complications (sinusitis, sinusitis, otitis media). With local intranasal application in therapeutic concentrations, oxymetazoline does not have a systemic effect, does not irritate and does not cause hyperemia of the nasal mucosa. The drug takes effect in 25 seconds,the duration of the action remains up to 12 hours.
Pharmacokinetics
With local intranasal administration, oxymetazoline does not have a systemic effect. The absorption of oxymetazoline is regarded as having no clinical significance.
Side effects
Nervous system disorders: Rarely: headache, insomnia, sedation, anxiety, irritability. Very rare: anxiety, drowsiness.
Gastrointestinal tract disorders: Rare: nausea.

Pregnancy and lactation

Drug interactions
OtrivinЃ Express is not recommended for patients receiving monoamine oxidase inhibitors at this time or who have received them within the previous 2 weeks. Concomitant use with tri- and tetracyclic antidepressants and sympathomimetic drugs can enhance the sympathomimetic effect of oxymetazoline.
Overdose
In case of oxymetazoline overdose, the clinical picture is characterized by: mydriasis, nausea, cyanosis, fever, tachycardia, cardiac arrhythmia, cardiac arrest, arterial hypertension, pulmonary edema, shortness of breath, mental disorders, hallucinations, convulsions. Symptoms associated with depression of the functions of the central nervous system may also appear, including Drowsiness, decreased body temperature, bradycardia, shock-like hypotension and loss of consciousness. Non-selective adrenergic antagonists such as phentolamine can be used to lower blood pressure. In severe cases, intubation and mechanical ventilation may be required. In case of moderate to severe overdose as a result of accidental oral ingestion, take activated charcoal and sodium sulfate (laxative).after a significant overdose, gastric lavage is prescribed. Conducting symptomatic therapy is shown. The use of vasopressors is contraindicated.
