pharmachologic effect
Peripheral vasodilator with a predominant effect on the venous vessels. Antianginal remedy. The mechanism of action is associated with the release of the active substance nitric oxide in the smooth muscles of the vessels. Nitric oxide activates guanylate cyclase and increases cGMP levels, which ultimately leads to relaxation of smooth muscle. Under the influence of glyceryl trinitrate, arterioles and precapillary sphincters relax to a lesser extent than large arteries and veins. This is partly due to reflex reactions, as well as less intense formation of nitric oxide from active substance molecules in the walls of arterioles. The action of nitroglycerin (glyceryl trinitrate) is mainly associated with a decrease in myocardial oxygen demand due to a decrease in preload (expansion of peripheral veins and a decrease in blood flow to the right atrium) and afterload (decrease in systemic vascular resistance). Promotes the redistribution of coronary blood flow to the ischemic subendocardial areas of the myocardium. Increases exercise tolerance in patients with coronary artery disease, angina pectoris. In heart failure, it contributes to the unloading of the myocardium, mainly by reducing the preload. Reduces pressure in the pulmonary circulation. In heart failure, it contributes to the unloading of the myocardium, mainly by reducing the preload. Pharmacokinetics
After oral administration, it is absorbed from the gastrointestinal tract, undergoes the first pass" effect through the liver.
With sublingual administration, this effect is absent, and the therapeutic concentration in the blood plasma is achieved within a few minutes.
It is metabolized in the liver with the participation of nitrate reductase.
From nitroglycerin metabolites, dinitro derivatives can cause severe vasodilation
perhaps it is they who determine the therapeutic effect of nitroglycerin (glyceryl trinitrate) when taken orally.
Plasma protein binding is 60%.
T1 / 2 after oral administration - 4 hours, with sublingual administration - 20 minutes, after intravenous administration - 1-4 minutes.
It is excreted mainly by the kidneys.
Indications for use
For sublingual and buccal use: relief and prevention of angina attacks
as an ambulance for acute myocardial infarction and acute left ventricular failure at the prehospital stage.
For oral administration: relief and prevention of angina attacks, rehabilitation treatment after myocardial infarction.
For intravenous administration: acute myocardial infarction, incl.
complicated by acute left ventricular failure
unstable angina
pulmonary edema.
For cutaneous use: prevention of angina attacks.
Contraindications for use
Shock, collapse, arterial hypotension (systolic blood pressure < 100 mm Hg, diastolic blood pressure < 60 mm Hg), acute myocardial infarction with severe arterial hypotension, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, cardiac tamponade, toxic pulmonary edema, increased intracranial pressure (including with hemorrhagic stroke, after a recent head injury), angle-closure glaucoma with high intraocular pressure, increased sensitivity to nitrates.
Dosage regimen
Applied sublingually, buccal, inside, skin, intravenous drip.
The dose and treatment regimen are set individually, depending on the indications, the specific clinical situation, and the dosage form used.
Side effect
From the side of the cardiovascular system: dizziness, headache, tachycardia, hyperemia of the skin, a feeling of heat, arterial hypotension
rarely (especially in case of overdose) - collapse, cyanosis.
From the digestive system: nausea, vomiting.
From the side of the central nervous system: rarely (especially in case of an overdose) - anxiety, psychotic reactions.
Allergic reactions: rarely - skin rash, itching.
Local reactions: slight itching, burning, redness of the skin.

Mode of application

Sublingual. < br> Nitroglycerin is recommended to be taken as directed by a doctor. < br> The tablet is kept under the tongue until it is completely absorbed, without swallowing, immediately after the onset of pain - 0.5–1 mg per dose. In many patients with stable angina pectoris, the effect comes from a lower dose (1 / 2–1 / 3 tablets), therefore, if the pain quickly passes, it is recommended to spit out the rest of the tablet that did not have time to dissolve. Usually the antianginal effect appears within 0.5–2 minutes 75% of patients notice improvement within the first 3 minutes, and another 15% - within 4-5 minutes. In the absence of antianginal action, within the first 5 minutes you need to take another 1 tablet. nitroglycerin. If there is no therapeutic effect after taking 2-3 tablets (capsules), you should immediately call a doctor. The duration of action after sublingual administration is about 45 minutes. With frequent attacks of angina pectoris, it is customary to prescribe prolonged nitroglycerin drugs. Tolerance to sublingual forms of nitroglycerin develops infrequently, nevertheless, when it occurs in some patients, the dose of the drug must be gradually increased, bringing it to 2-3 tablets.
