Pharmacological action

Triftazine is an antipsychotic, from the group of piperazine derivatives of phenothiazine. It has a pronounced antipsychotic and antiemetic effect, has a-adrenolytic and weak anticholinergic and sedative effects. The antipsychotic effect is combined with a moderate stimulating effect (in small doses). The mechanism of action of metronidazole is the biochemical recovery of the 5-nitro group of metronidazole by intracellular transport proteins of anaerobic microorganisms and protozoa. The reduced 5-nitro group of metronidazole interacts with the DNA of the cell of microorganisms, inhibiting the synthesis of their nucleic acids, which leads to the death of microorganisms.

Metronidazole is an effective antimicrobial and antiprotozoal agent with a wide spectrum of action. The drug is highly active against Trichomonas vaginalis, Giardia intestinalis, Entamoeba histolytica, Lamblia intestinalis, as well as against obligate anaerobes (spore-and non-spore-forming) - Bacteroides spp. (B. fragilis, B. ovatus, B. distasonis, B. thetaiotaomicron, B. vulgatus), Fusobacterium spp., Clostridium spp., Peptostreptococcus spp., which is achieved after a single dose of the equivalent dose of metronidazole inside.

Metronidazole is almost completely metabolized in the liver to form inactive derivatives.

T1 / 2 - 8 hours. Metronidazole is excreted mainly in urine.

With intravaginal administration, chlorchinaldol is not significantly absorbed.

Indications

Infectious-inflammatory diseases of the vagina caused by pathogens sensitive to the drug.

Use during pregnancy and lactation

Contraindicated in pregnancy. At the time of treatment should stop breastfeeding.

Dosage and Administration

Intravaginally. The tablet is injected deep into the vagina, preferably in the evening before bedtime, 1 tablet. Once a day for 7-10 days. Treatment begins 2-4 days after menstruation.

If necessary, a second course of treatment is possible.

Side effects

Itching, burning, pain and irritation in the vagina thick, white, mucous discharge from the vagina odorless or with a faint smell, rapid urination after drug withdrawal - development of vaginal candidiasis burning sensation or irritation of the penis in the sexual partner headache, dizziness, leukopenia or leukocytosis, nausea, change in taste, metallic taste in the mouth, dry mouth, decreased appetite, spastic pain in the abdominal cavity, vomiting, constipation or diarrhea allergic reactions: urticaria, itching of the skin, rash.

In rare cases, red-brown urine may stain due to the presence of a water-soluble pigment resulting from the metabolism of metronidazole.

Drug Interaction

Alcohol intake should be avoided during treatment (similar to disulfiram causing alcohol intolerance).

Caution should be exercised when co-administered with certain medications.

Warfarin and other indirect anticoagulants. Metronidazole enhances the effect of indirect anticoagulants, leading to an increase in the time of prothrombin formation.

Disulfiram. The development of neurological symptoms (depression of consciousness, the development of mental disorders), so Ginalgin should not be prescribed to patients who have been taking disulfiram for the past two weeks.

Cimetidine inhibits the metabolism of metronidazole, which may increase its serum concentration and increase the risk of side effects.

Lithium preparations increase the concentration of the latter in plasma.

The effects of Ginalgin under the influence of barbiturates decrease because inactivation of metronidazole in the liver is accelerated.

When taken with lithium, the plasma concentration of the latter may increase.

It is possible to reduce the effect of Ginalgin under the influence of barbiturates, since inactivation of metronidazole in the liver is accelerated.
