Pharmacological action
Pharmacodynamics
Analgin has analgesic, antipyretic and anti-inflammatory effects.
Inhibits the activity of cyclooxygenase, reduces the formation of prostaglandins, endoperoxides, active oxygen radicals.
Reduces the conduction of pain impulses along the ganglio-bulbar and spinothalamic tracts, increases the threshold of excitability of the thalamic centers of pain sensitivity. Increases heat dissipation.
Pharmacokinetics
When taken orally, it is rapidly and completely absorbed. The onset of action ranges from 30 to 60 minutes after ingestion and 30 minutes after parenteral administration and reaches a maximum after 2 hours.
Hydrolyzes in the intestinal wall with the formation of an active metabolite - unchanged metamizole in the blood (only after intravenous administration, its insignificant concentrations are found in plasma).
The level of binding of the active metabolite to proteins is 50-60%. Destroyed in the liver. Excretion passes through the kidneys.
Indications
- acute pain after trauma or surgery
- acute pain due to muscle spasms of the internal organs
- pain due to tumor process
- other acute or chronic intense pains when other therapeutic measures are impossible
- fever, in the absence of effect from other antipyretic drugs.
Contraindications
hypersensitivity to pyrazolone derivatives (butadione, tribuzon)
bronchial asthma
diseases accompanied by bronchospasm
“aspirin asthma”
inhibition of hematopoiesis (agranulocytosis, neutropenia, leukopenia)
severe liver or kidney dysfunction
blood disease hereditary disease (especially in the first trimester and in the last 6 weeks)
lactation period.
Precautions - infant age (up to 3 months).
Use during pregnancy and lactation
Contraindicated in pregnancy (especially in the first trimester and in the last 6 weeks). At the time of treatment should stop breastfeeding.
Special instructions
In patients suffering from bronchial asthma and pollinosis, the development of hypersensitivity reactions is possible.
With prolonged use (more than 7 days), it is necessary to control the picture of peripheral blood.
Do not use to relieve acute abdominal pain (until the cause is clarified).
When treating children under 5 years old and patients receiving cytotoxic drugs, metamizole sodium should be administered only under medical supervision.
Special care is required when prescribing to patients who abuse alcohol.
Children under 2 years of age are recommended to use children's dosage forms.
Children and adolescents under 18 years of age should only be used as directed by a doctor.
Composition
Active ingredient - metamizole sodium? 500 mg
excipients:
potato starch,
talc,
sodium lauryl sulfate,
calcium stearate.
Dosage and administration of
Inside, for adults - 1 tablet. 2-3 times a day after meals. The maximum single dose is 1 g (2 tablets), daily - 3 g (6 tablets). With prolonged (more than a week) use of the drug, it is necessary to control the picture of peripheral blood and the functional state of the liver.
Children are prescribed 5-10 mg / kg of body weight 3-4 times a day for no more than 3 days (pre-grinding tablet).
Do not use (without consulting a doctor) for more than 3 days when prescribed as an antipyretic and more than 5 days when prescribed as an anesthetic.
Side effects
Allergic reactions: skin rash, Quincke's edema rarely - anaphylactic shock, toxic epidermal necrolysis (Lyell's syndrome), in rare cases - Stevens-Johnson syndrome.
With prolonged use, leukopenia, thrombocytopenia, agranulocytosis may occur.
With a tendency to bronchospasm, an attack may provoke a decrease in blood pressure.
Possible impaired renal function, oliguria, anuria, proteinuria, interstitial nephritis, red staining of urine due to the release of the metabolite.
About all side (unusual) effects, including not listed above, you should inform your doctor and stop taking the drug.
Drug Interactions
The simultaneous use of Analgin with other non-narcotic analgesics can lead to a mutual increase in toxic effects.
Tricyclic antidepressants, contraceptives for oral administration and allopurinol disrupt the metabolism of metamizole in the liver and increase its toxicity.
Barbiturates and phenylbutazone weaken the effect of analgin.
Analgin enhances the effects of alcohol-containing drinks.
X-ray contrast agents, colloidal blood substitutes, and penicillin should not be used during treatment with metamizole.
Metamizole, displacing oral hypoglycemic drugs, indirect anticoagulants, corticosteroids and indomethacin from a protein bond, increases their activity.
The simultaneous use of analgin with cyclosporine reduces the level of the latter in the blood. Thiamazole and sarcolisin increase the risk of developing leukopenia. The effect is enhanced by codeine, propranolol (slows down inactivation).
Sedatives and tranquilizers enhance the analgesic effect of analgin.
Overdose
Symptoms: long-term use of the drug (more than 7 days) in large doses - nausea, vomiting, gastralgia, oliguria, hypothermia, decreased blood pressure, tachycardia, shortness of breath, tinnitus, drowsiness, delirium, disturbances of consciousness, agranulocytosis, hemorrhagic syndrome, acute renal and / or hepatic failure, convulsions, paralysis of respiratory muscles.
Treatment: cause vomiting, gastric lavage, take salt laxatives, activated charcoal. In the conditions of medical institution - carrying out forced diuresis, hemodialysis, in the development of convulsive syndrome - in / in the introduction of diazepam and barbiturates.
