Pharmacological action
Anabolic, antihypoxic, antithyroid, stimulating regeneration, stimulating fat metabolism.
Belongs to the group of vitamins B (BT - "growth vitamin"). It is a cofactor of metabolic processes that ensure the maintenance of CoA activity. It has an anabolic effect, reduces basic metabolism, slows down the breakdown of protein and carbohydrate molecules. It promotes penetration through the membranes of mitochondria and the breakdown of long chain fatty acids (palmitic and others. ) with the formation of acetyl-CoA (necessary to ensure the activity of pyruvate carboxylase in the process of gluconeogenesis, the formation of ketone bodies, the synthesis of choline and its esters, oxidative phosphorylation and the formation of ATP). It has a fat-mobilizing effect due to the presence of three labile methyl groups. Competitively displacing glucose, it includes a fatty acid metabolic shunt, the activity of which is not limited by oxygen (unlike aerobic glycolysis), therefore it is effective in acute brain hypoxia and other critical conditions. Causes slight depression of the central nervous system, increases the secretion and enzymatic activity of digestive juices (gastric and intestinal), improves the absorption of food. Reduces excess body weight and reduces muscle fat. Raises the threshold of resistance to physical activity, leads to the elimination of post-loading acidosis and, as a consequence, the restoration of performance after prolonged exhausting physical exertion. Increases glycogen reserves in the liver and muscles, contributes to its more economical use. It has a neurotrophic effect, inhibits the development of apoptosis, limits the affected area and restores the structure of the nervous tissue. In the blood plasma of adults and older children, endogenous carnitine is detected at a concentration of 50 μmol / L. When taken orally, it is well absorbed, the plasma level reaches a maximum after 3 hours and remains in the therapeutic range for 9 hours. When injected intramuscularly, it is found in plasma for 4 hours, after intravenous administration it disappears from the blood after 3 hours. It easily penetrates the liver and myocardium, more slowly - into the muscles. It is excreted by the kidneys mainly in the form of acyl ethers.
Indications
Consequences of birth injury and asphyxiation of the newborn, hypotrophy and hypotension of the newborn, respiratory distress syndrome in the newborn, nursing of premature infants who are fully parenteral fed, and children who undergo hemodialysis, a syndrome complex similar to Reye’s syndrome (hypoglycemia, hypoketonemia, coma), which develops in children with valproic acid and is underweight in children and adolescents under 16 years old and other "organic" acididemia, primary (genetic) and secondary carnitine deficiency, including in patients with chronic renal failure on hemodialysis, mild forms of thyrotoxicosis, exogenous constitutional obesity, psoriasis, seborrhea, skin forms of scleroderma, cardiomyopathy, myocarditis, coronary heart disease (angina pectoris, acute myocardial infarction, post-infarction), cardiomyopathy, cariopia chronic hypoacid gastritis, chronic pancreatitis with impaired exocrine function, liver disease, neurasthenia, traumatic encephalopathy, physical exhaustion, intense physical activity in athletes (as an anabolic and adaptogen), prevention of cardiotoxicity in the treatment of anthracyclines.
Contraindications
Hypersensitivity.
Composition of
Levocarnitine.
Dosage and administration
Inside, iv. Inside, 30 minutes before meals, diluting with water, tea, jelly, adults 2-3 times a day for 1/2 to 2 teaspoons of a 20% solution or 1-2 chewable tables. For ease of use in newborns, 3 ml of a 20% solution of 200 ml of a 5% glucose solution is diluted before use and the resulting solution is prescribed 10–20 ml (2–4 teaspoons) 2-3 times a day 30 minutes before feeding. At the end of the neonatal period up to 1 year, 10 drops of a 20% solution are prescribed, for children 1–6 years old - 14 drops, 6–12 years old - 1/4 teaspoon 2-3 times a day. In / in, slowly, with acute myocardial infarction in a daily dose of 100-200 mg / kg in 4 divided doses or as a continuous infusion for 48 hours, followed by a 2-fold dose reduction for patients on chronic hemodialysis - 2 g once immediately after completion of the next session.
Side effects
Pain in the epigastric region, dyspeptic symptoms, muscle weakness.
Drug interaction
Glucocorticoids promote accumulation in tissues (except the liver), other anabolics enhance the effect.
