Pharmacological action
Allapinin is a class I antiarrhythmic drug. Lappaconitin is an alkaloid obtained from the wild-growing perennial plant of aconite of the white-leaved family of buttercups.
Allapinin (lappaconitine) has a membrane-stabilizing effect, blocking the fast sodium channels of the membranes of cardiomyocytes.
Allapinin slows down the atrioventricular and intraventricular conduction, shortens the effective and functional refractory periods in the atria and ventricles. It has a moderate antispasmodic, coronary expansion, anticholinergic and sedative effect. Bioavailability is 39% - is subjected to intensive presystemic biotransformation.
With the on / in the introduction and ingestion, the elimination half-life is 1-1.2 hours. Penetrates through the BBB. One of the three main metabolites lowers blood pressure.
When administered, the effect develops after 40-60 minutes, reaches a maximum after 4-5 hours and lasts 8 hours or more. In terms of iv administration, the antiarrhythmic effect manifests itself after 15-20 minutes and lasts 6-8 hours (with a maximum after 2 hours).
Elongation of half-life noted with prolonged use, renal failure (2-3 times), cirrhosis (3-10 times).
Contraindications
hypersensitivity, atrioventricular block (AV) II and higher, severe liver and kidney failure pregnancy (prescribed only for health reasons)
Special instructions
In case of headache, dizziness, diplopia, the dose should be reduced. Repeated parenteral administration is allowed only after 6 hours.
With the appearance of sinus tachycardia with long-term use, beta-blockers are indicated (low doses).
Prescribing to patients with AV block I degree, sick sinus syndrome and severe heart failure is possible only in a hospital with medical supervision and ECG monitoring.
With caution (mainly for health reasons) is prescribed in the first 3 months of pregnancy.
Composition
1 tablet contains:
Active ingredient:
allapinin - 25 mg
Excipients:
sucrose (sugar),
potato starch,
calcium stearate (calcium stearic acid).
Dosage and administration
Inside, with warm water (tablets must be ground beforehand), 30 minutes before meals.
First prescribed at 0.025 g every 8 hours in the absence of effect - 0, 025 g every 6 hours.
Possible further increase in dose to 0.05 g per dose every 6-8 hours.
The course depends on the condition of the patient and the nature of the rhythm disturbance and treatment can last up to several months. The highest single dose - 0.15 g, daily - 0.3 g.
Side effects
Dizziness, headache, dizziness, facial flushing, diplopia, ataxia, electrocardiographic changes, allergic reactions.
With prolonged use, sinus tachycardia may appear.

Overdose
Symptoms: the drug has a small breadth of therapeutic effect, therefore severe intoxication can easily occur (especially with the simultaneous use of other antiarrhythmic drugs): prolongation of the PR and QT intervals, expansion of the QRS complex, increase in the amplitude of the T wave, bradycardia, sinoatrial blockade, AV -blockade, asystole, paroxysms of polymorphic ventricular tachycardia, decreased myocardial contractility, a marked decrease in blood pressure, dizziness, blurred vision, headache, gastrointestinal upset.
Treatment: conduct symptomatic therapy.
For treatment of ventricular tachycardia, class I A and I C antiarrhythmic drugs should not be used. Sodium bicarbonate can eliminate QRS complex expansion, bradycardia and arterial hypotension.
