Pharmacological action
Peripheral vasodilator with a primary effect on venous vessels. Cardiket has an antianginal and hypotensive effect.
The mechanism of action is associated with dilatation of blood vessels (mainly post-capillary) and large arteries. Dilation of the vessels of the postcapillary bed leads to venous deposition of blood and a decrease in venous return of blood to the heart. As a result of reducing the filling of the left ventricle (reducing preload), the oxygen demand of the myocardium decreases, which contributes to an increase in capillary blood flow, opening the lumen of previously non-functioning intramural coronary vessels, as well as improving the supply of oxygen to the ischemic subendocardial region.
Dilatation of precapillary vessels leads to a decrease in systolic pressure in the aorta and OPSS (decrease in afterload). As a result, the cardiac output of the left ventricle and the myocardial oxygen demand are reduced.
Direct dilatation of the coronary vessels and a slight decrease in the resistance of the coronary vessels improve myocardial perfusion and oxygen supply without the development of a robbery syndrome.
Indications
prevention of angina attacks
rehabilitation treatment after myocardial infarction
pulmonary hypertension
chronic heart failure (as part of combination therapy).
Contraindications
acute myocardial infarction
acute circulatory failure (shock, vascular collapse)
severe arterial hypotension (systolic blood pressure less than 90 mm Hg).
Use during pregnancy and lactation
The use of the drug Cardiket during pregnancy is possible only if the intended benefit to the mother outweighs the potential risk to the fetus.
If necessary, the appointment of the drug during lactation should decide on the termination of breastfeeding.
Dosage and administration
Dose is selected individually, since it depends on the stage of the disease and the individual need for nitrates. Tablets of 20 mg are prescribed 1 tab. 2-3 times / day, Cardiket should be taken after a meal, without chewing and drinking with a small amount of liquid. When prescribing more than 1 tablet / day, the interval between doses should be at least 8 hours (to ensure the full effect of the drug).
Side effects
From the cardiovascular system: at the beginning of treatment - severe headache (due to dilation of the venous channel of the brain vessels) possibly - tachycardia, orthostatic hypotension.
From the side of the central nervous system: drowsiness.
From the gastrointestinal tract: in some cases - nausea, vomiting.
Dermatological reactions: in some cases, erythematous rashes.
Drug Interaction
With the simultaneous use of Cardiket, a retard with antihypertensive drugs, cyclic antidepressants, MAO inhibitors may enhance its antihypertensive effect.
When co-administered with Cardiket, retard potentiates the effect of dihydroergotamine and ethanol.
Overdose
Symptoms: headache, palpitations, dizziness, hypotension, nausea, vomiting.
Treatment: symptomatic.
Storage conditions
The drug should be stored at a temperature not exceeding 25 РC.
