Pharmacological action
Maltofer - a preparation containing iron, in the form of a polymaltose complex of ferric hydroxide. This macromolecular complex is stable in the gastrointestinal tract and does not release iron in the form of free ions.
Maltofer is similar in structure to the natural combination of iron and ferritin. Due to this similarity, iron (III) from the intestine enters the bloodstream through active absorption.
It is this property of Maltofer that explains the impossibility of drug poisoning, unlike simple iron salts, which are absorbed by a concentration gradient.
Absorbed iron is retained in a ferritin-bound form, mainly in the liver. Later, in the bone marrow, it is included in the composition of hemoglobin. Iron, which is part of iron (III) - the hydroxide of the polymaltose complex, does not have the prooxidant properties that are inherent in simple salts of iron (II).
There is a correlation between the degree of iron deficiency and the amount of absorbed iron (the higher the iron deficiency, the better absorption). The maximum absorption of iron occurs in the duodenum and jejunum.
Folic acid - a vitamin of group B, stimulates erythropoiesis, participates in the synthesis of amino acids, nucleic acids, purines, pyrimidines, in the exchange of choline.
Maltofer Foul does not cause staining of teeth.
Indications
Treatment of latent and clinically severe iron deficiency (anemia).
Prevention of iron and folic acid deficiency, including before, during and after pregnancy (lactation).
Contraindications
Excess iron in the body (hemochromatosis, hemosiderosis).
Disruption of iron utilization (lead anemia, sidereal anemia, thalassemia)
Non-iron deficiency anemia (e.g. hemolytic anemia or megaloblastic anemia caused by vitamin B12 deficiency)
Special instructions
When prescribing the drug to patients with diabetes, it should be borne in mind that 1 tablet contains 0.04 bread units.
In controlled trials in pregnant women after the first trimester of pregnancy, there was no adverse effect of the drug on the mother and fetus. There is no data on the undesirable effect of the drug on the fetus during the first trimester of pregnancy.
Dosage and Administration
Dosage and duration of therapy depend on the severity of iron deficiency. The daily dose of the drug can be taken once a day. Maltofer Foul tablets should be taken during or immediately after a meal. They can be chewed or swallowed whole.
Treatment of clinically severe iron deficiency (iron deficiency anemia): 1 tablet 1-3 times a day for 3-5 months until the hemoglobin level of the blood normalizes. After this, the drug should be continued for a few more months in order to restore iron stores in the body (1 tablet per day).
Pregnant women should take 1 tablet Maltofer Foul 2-3 times a day until hemoglobin normalizes. Then, therapy should be continued at a dosage of 1 tablet per day, at least until delivery to restore iron stores.
Duration of therapy should be at least 2 months.
In the case of clinically severe iron deficiency, normalization of hemoglobin level occurs only 2-3 months after the start of treatment.
For the treatment of latent iron deficiency and for the prevention of iron and folic acid deficiency, patients should take 1 tablet of the drug per day.
Side effects of
Occasionally, signs of irritation of the gastrointestinal tract, such as, for example, a feeling of fullness and pressure in the epigastric region, nausea, constipation or diarrhea, as well as allergic reactions to folic acid, may be noted.
The dark color of the stool is due to the removal of non-absorbed iron and has no clinical significance.
