Pharmacological action
Atarax is a piperazine derivative, has moderate anxiolytic activity and also has a sedative, antiemetic, antihistamine and M-anticholinergic effect. It blocks the central m-choline and H1-histamine receptors and inhibits the activity of certain subcortical zones. It does not cause mental dependence and addiction. The clinical effect occurs 15-30 minutes after ingestion of the drug.
Has a positive effect on cognitive abilities, improves memory and attention. It relaxes skeletal and smooth muscles, has bronchodilating and analgesic effects, a moderate inhibitory effect on gastric secretion. Hydroxyzine significantly reduces itching in patients with urticaria, eczema and dermatitis. With prolonged use, there is no marked withdrawal and cognitive impairment syndrome. Polysomnography in patients with insomnia and anxiety clearly demonstrate an increase in the duration of sleep, a decrease in the frequency of night awakenings after a single or repeated administration of hydroxyzine at a dose of 50 mg. A decrease in muscle tension in patients with anxiety was noted when taking the drug at a dose of 50 mg 3 times / day.
Indications
- anxiety (including generalized anxiety, adaptation disorders), psychomotor agitation, a feeling of internal tension, increased irritability in neurological, mental and somatic diseases, chronic alcoholism
- withdrawal syndrome in patients with chronic alcoholism, accompanied by psychomotor - as a sedative during
premedication - skin itching (as symptomatic therapy).
Contraindications
- porphyria
- pregnancy
- childbirth
- lactation (breastfeeding)
- hypersensitivity to any component of the drug, cetirizine and other piperazine derivatives, aminophylline or these.
It is not recommended to prescribe Atarax tablets to patients with hereditary galactose intolerance, as well as impaired glucose and galactose absorption, because the composition of the tablets includes lactose.
With caution, the drug should be prescribed for myasthenia gravis, prostatic hyperplasia with clinical manifestations, difficulty urinating, constipation, with increased intraocular pressure, dementia, a tendency to convulsive seizures, with a predisposition to the development of arrhythmia, with the simultaneous use of drugs, possessing arrhythmogenic effect, with simultaneous use with drugs that depress the central nervous system, MAO inhibitors or anticholinergics.
Use during pregnancy and lactation
Atarax is contraindicated in pregnancy, during labor.
If necessary, use of Atarax during lactation should stop breastfeeding.
Special instructions
If allergy testing is necessary, Atarax should be discontinued 5 days before the study.
Patients taking Atarax should refrain from drinking alcohol.
Influence on the ability to drive vehicles and control mechanisms
Patients taking Atarax, if necessary, control cars and mechanisms, should be warned that the drug may affect concentration and speed of psychomotor reactions.

Side effects of
Side effects associated with anticholinergic action: rarely (mainly in elderly patients) - dry mouth, urinary retention, constipation, poor accommodation.
From the side of the central nervous system: drowsiness, general weakness (especially at the beginning of treatment), headache, dizziness.
From the cardiovascular system: arterial hypotension, tachycardia.
From the digestive system: nausea, changes in liver function tests.
Other: increased sweating, allergic reactions, fever, bronchospasm.
When using the drug in recommended doses, no clinically significant respiratory depression was noted.
Side effects observed with Atarax are usually mild, transient, and disappear within a few days from the start of treatment or after a dose reduction.
Drug Interactions
Atarax potentiates the action of central nervous system depressants, such as opioid analgesics, barbiturates, tranquilizers, sleeping pills, ethanol (which requires dose adjustment with the simultaneous administration of these drugs and Atarax).
Atarax with simultaneous use reduces the pressor effect of epinephrine and anticonvulsant activity of phenytoin, and also inhibits the action of betahistine and cholinesterase inhibitors.
With the simultaneous use of Atarax does not affect the activity of atropine, belladonna alkaloids, cardiac glycosides, antihypertensive drugs, H2 receptor blockers and other m-anticholinergic drugs.
Hydroxyzine is an inhibitor of the isoenzyme 2D6 of the cytochrome P450 system and, when used in high doses, can cause interaction with CYP2D6 substrates. Since hydroxyzine is metabolized in the liver, we can expect an increase in its concentration in the blood when co-administered with inhibitors of liver enzymes.
overdose
Symptoms: increased anticholinergic effects, depression or paradoxical CNS stimulation, nausea, vomiting, involuntary motor activity, hallucinations, impaired consciousness, arrhythmia, arterial hypotension, rarely, tremor, that occur when the recommended doses are significantly exceeded.
Treatment: if there is no spontaneous vomiting, it should be caused artificially or by gastric lavage. Conduct general activities aimed at maintaining vital functions of the body, and monitor the patient until the symptoms of intoxication disappear within the next 24 hours
Norepinephrine or metaramenol is prescribed if necessary to obtain a vasopressor effect. Epinephrine should not be used because Atarax interferes with its pressor action.
There is no specific antidote. Hemodialysis is ineffective.
