Pharmacological action
Plasma substitute, hydroxyethylated starch (HES). HES is a high molecular weight compound consisting of polymerized glucose residues, which is obtained by hydroxyethylation of amylopectin, a natural polysaccharide found in potato starch and waxy maize.
Amylopectin is rapidly hydrolyzed in the blood its presence in the bloodstream is about 20 minutes.
Amylopectin is hydroxyethylated to increase stability and increase the duration of action. The depth of this process is characterized by the degree of substitution.
The intensity and duration of the volume-replacing action of HES preparations are determined by the molecular weight and degree of substitution of the HES substance. The higher the degree of substitution, the longer the HES is retained in the circulating blood. With a C2 / C6 ratio of more than 8, HES is metabolized in the body much more slowly than with a ratio of less than 8.
Pentac starch has a molecular weight of about 200,000 daltons and a degree of substitution of about 0.5, the C2 / C6 ratio, which determines the characteristics of substitution, is 6.
Improves rheological properties blood by reducing hematocrit, reduces plasma viscosity, reduces red blood cell aggregation, helps restore impaired microcirculation. It is deposited in the cells of RES, which does not have clinical significance.

Indications
Prevention and treatment of relative and absolute hypovolemia, shock resulting from bleeding or trauma, intraoperative blood loss, burns, sepsis Acute normovolemic hemodilution, therapeutic hemorrhaging
Contraindications
Hyperhydration, hypervolemia, decompensated heart failure, renal failure with oliguria or anuria, cardiogenic pulmonary edema, intracranial bleeding, severe bleeding disorders, hypersensitivity to HES.
Recommendations for use
Install individually depending on age, indications, clinical situation.
Enter in the form of in / in infusion.
Special instructions
Used with caution in hemorrhagic diathesis, dehydration (prior corrective therapy is required). It should be borne in mind that too rapid IV administration, as well as the use of HES in large doses, can cause acute volumetric overload of the circulatory system.
In the course of treatment, it is necessary to control the ionogram of blood plasma, bcc, renal function, monitor the sufficient introduction of fluid. With the development of hypersensitivity reactions, you should immediately stop the introduction of HES and take the necessary urgent measures.
When using HES, an increase in serum amylase activity is possible, which is not associated with the clinical manifestations of pancreatitis.
Pentacarchum can be used for leukapheresis. Pentacarchum does not have a mutagenic effect.
Composition
Infusion solution 6% 1 l
hydroxyethyl starch 200 / 0.5 60 g

Allergic reactions: possible urticaria, anaphylactic reactions rarely - persistent, but reversible skin itching.
active substance
Hydroxyethyl
