Pharmacological action
Albumin is an integral part of the protein fraction of human blood. The drug is produced from donor plasma, tested for the absence of antibodies to human immunodeficiency viruses (HIV-1, HIV-2), hepatitis C virus and hepatitis B surface antigen. Albumin in the body performs the following main functions: hemodynamic (maintaining colloidal oncotic blood pressure ), sorption-transport and function of the main protein reserve of the body.
10% albumin solution is a mild hyperoncotic solution, supporting oncotic pressure of the circulating blood. When applied to the vascular bed, fluid from the interstitial space is attracted and retained in it, which increases and stabilizes blood pressure.
Albumin binds and inactivates toxins, transports pigments (bilirubin), fatty acids, certain metal ions, drugs, improves tissue perfusion in diseases accompanied by hypoproteinemia, and has a positive effect on protein synthesizing liver function.
Indications
Hypoalbuminemia of various origins (a decrease in plasma albumin below 30 g / l, or a decrease in total protein below 50 g / l):
Shock (hemorrhagic, traumatic, thermal).
Acute blood loss (decrease in circulating blood volume by more than 25-30%).
Purulent-septic state.
Diseases of the liver (with impaired albumin synthesizing function).
Kidney disease (nephritis, nephrotic syndrome).
Burn disease.
Operations using cardiopulmonary bypass.
Therapeutic plasmapheresis.
Hemolytic disease of the newborn during an exchange blood transfusion.
Preoperative hemodilution and harvesting of autologous blood components.
Edema of the brain (10 and 20% albumin solution).
The main indicator for the use of 5-10% albumin solutions is hypovolemia, 20% hypoproteinemia.
Contraindications
Hypersensitivity.
Severe forms of heart failure (due to the possible occurrence of acute heart failure).
Thrombosis.
Severe form of hypertension.
Continuous internal bleeding.
Hypervolemia.
Pulmonary edema.
Severe anemia.
Edema of the brain.
With caution is prescribed for: renal failure, bronchial asthma, allergic rhinitis, Quincke's edema.
Use during pregnancy and lactation
In pregnancy, the use of human albumin is possible only in cases of emergency.
Composition of
In 1 ml of solution for infusion contains:
Active ingredient: human albumin 100 mg
Excipients: sodium caprylic acid, sodium chloride, water for injection.
Dosage and administration
10% albumin solution is administered intravenously or by drop.
A single dose of the drug depends on the concentration of the albumin solution, the initial condition and the age of the patient. Solutions of albumin 10% are administered in a dose of 200-300 ml.
The rate of administration of a 10% albumin solution is not more than 40 drops per minute.
Inkjet administration of albumin solutions is acceptable for shocks of various origins to rapidly increase blood pressure.
In pediatric practice, the dosage of albumin solutions is calculated taking into account the concentration in milliliters per kilogram of body weight (not more than 3 ml / kg of body weight of the child).
Side effects of
Administration of 10% albumin solution, as a rule, is not accompanied by adverse reactions and complications, sometimes it is possible to develop:
Allergic reactions of varying severity (urticaria and others).
Hyperthermia.
Pain in the lumbar region.
Drug Interaction
Co-administration of albumin with ACE inhibitors increases the risk of hypotension.
Overdose
Hypervolemia (development of pulmonary edema, increased arterial and central venous pressure) may occur with an overdose or over-administration of the drug.
At the initial signs of anaphylactic reaction or signs of overdose (headache, shortness of breath, asphyxiation), the transfusion of albumin should be stopped immediately and, without removing the needles from the vein, the appropriate preparations (glucocorticosteroids, vasopressor agents if available) should be administered.
