Pharmacological action of

Replenishes the deficiency of folic acid, stimulates erythropoiesis.

Folic acid - folacin, water soluble folate, vitamin B9. In the body, it is converted to tetrahydrofolic acid, which is necessary for the maturation of megaloblasts and their transformation into normoblasts. With its deficiency, a megaloblastic type of hematopoiesis develops. It occupies an important place in the exchange of purines and pyrimidines, the synthesis of nucleic acids, the metabolism of amino acids (glycine, methionine and histidine). After ingestion, folic acid, combining in the stomach with Castle's internal factor (specific glycoprotein), is absorbed in the upper part of the duodenum. Almost completely binds to plasma proteins. It is activated in the liver by the action of the enzyme dihydrofolate reductase, turning into tetrahydrofolate acid. Cmax in the blood is reached after 30-60 minutes. It is excreted by the kidneys both unchanged and in the form of metabolites.

Contraindications

Hypersensitivity to the components of the drug (PM).

Malignant anemia and other conditions associated with vitamin B12 deficiency due to the fact that folic acid monotherapy can accelerate the onset of subacute degenerative changes in the spinal cord in this case. Taking folic acid as monotherapy for more than 3 months in such individuals leads to the development of cobalamin neuropathy. Shorter courses do not cause this complication. In the elderly, in this regard, before starting folic acid therapy, cobalamin absorption must be evaluated.

Folate-dependent neoplasms, except in cases where severe megaloblastic or macrocytic anemia is a complication of ongoing antitumor therapy.

Special instructions

In case of pernicious anemia, folic acid should be used only in conjunction with cyanocobalamin, since folic acid, stimulating hematopoiesis, does not prevent the development of neurological complications (including funicular myelosis). Long-term use of folic acid (especially in high doses) is not recommended because of the risk of a decrease in the concentration of cyanocobalamin in the blood.

Composition

Each tablet contains: active ingredient: folic acid - 1 mg

excipients: powdered sugar, potato starch, stearic acid.

Dosage and Administration

Inside. Before eating. It is allowed to grind the tablet before taking it for children and people who have difficulty swallowing.

Adults (including the elderly): With drug deficiency of folic acid - 5 mg per day for 4 months with malabsorption - up to 15 mg daily may be used.

For the prevention of anemia in chronic hemolytic status: 5 mg every 1 to 7 days, depending on the exacerbations of the disease.

Children with hemolytic anemia and metabolic disorders

In children under the age of 4 years, it is allowed to use tablets in pre-ground form in powder form. Doses of folic acid in children are: at the age of 1 year, 0.5 mg / kg (? Tablets per 1 kg of weight) 1 time per day, but not more than 5 mg / day, at the age of 1 year to 12 years - 2, 5 - 5 mg / day over 12 years - 5 - 10 mg / day 1 time per day.

For the treatment of macrocytic (folic acid deficiency) anemia is prescribed: for adults and children of any age, the initial dose is up to 1 mg / day (1 tablet). Daily doses of more than 1 mg do not enhance the hematological effect, and most of the excess folic acid is excreted unchanged in the urine. In resistant cases, higher doses may be required. Initial doses are used for about 14 days or until a hematological and clinical response is obtained, then they are transferred to maintenance treatment. The total duration of therapy may be about 4 months.

Supportive treatment: for children under 4 years: for infants - 0.1 mg / day, children under 4 years old - up to 0.3 mg / day for children over 4 years old and adults - 0.5 mg each (? tablets) during pregnancy and lactation - 1 mg / day, but not less than 0.1 mg / day.

With established folic acid deficiency during pregnancy: 5 mg per day throughout pregnancy. For the prevention of neural tube developmental defects in the presence of malformations in the anamnesis: 4 - 5 mg daily for 1 month before the planned conception and for 3 months after it. In the absence of a history of neural tube malformations (or other predisposing factors): 0, 5 mg (? Tablets) daily for 10 to 12 weeks after the last menstruation. During pregnancy, the administration of folic acid in doses above 0.4 mg / day can be carried out only after malignant (pernicious) anemia associated with a deficiency of vitamin B12 is excluded.

Side effects

Allergic reactions: bronchospasm, erythema, fever, skin rashes.

Drug interaction

When used simultaneously with chloramphenicol, neomycin, polymyxins, tetracyclines, folic acid absorption decreases.

With the simultaneous use of folic acid reduces the effects of phenytoin, primidone, PASK, sulfasalazine, hormonal contraceptives for oral administration, chloramphenicol.

Folic acid increases phenytoin metabolism.
