Indications

Parenteral nutrition (as a source of energy and essential fatty acids). Intralipid is especially necessary in conditions associated with hypercatabolism: sepsis, polytrauma, severe burns, etc.

Contraindications

The acute stage of shock, hypersensitivity to the components of the drug, severe lipid metabolism disorders, for example, with pathological

hyperlipidemia.

Caution: diseases that cause lipid metabolism disorders: severe renal and / or liver failure, decompensated diabetes mellitus, pancreatitis, hypothyroidism (with concomitant hypertriglyceridemia), soy protein allergy (only after allergic tests).

Composition

Active ingredients: purified soybean oil 200 g

Excipients: egg yolk phospholipids - 12 g glycerol (anhydrous) - 22 g sodium hydroxide (for pH adjustment to 8) - qs water for injection - up to 1000 ml.

Indicators: osmolality - 350 mosmol / kg water energy value - 2000 kcal / l (8400 kJ / l) pH - 7.5–8.5

Dosage and Administration

Dose and route of administration should be determined by the ability to eliminate intralipid.

Adults. The maximum daily dose of 3 g of triglycerides per 1 kg of body weight per day (15 ml of Intralipid 20%). The infusion time of 500 ml of Intralipid 20% - at least 5 hours. Intralipid can cover up to 70% of energy needs.

Newborns and young children. The daily dose is from 0.5 to 4.0 g of triglycerides per 1 kg of body weight per day, which corresponds to 2.5-20 ml of Intralipid 20%. The infusion rate is not more than 0.15 g / kg / h.

Drug Interaction

Co-administration of Intralipia with Vitalipid Adult and Vitalipid Pediatric, Soluvit is allowed.

Mixing solutions should be carried out under aseptic conditions.

Heparin causes a transient increase in plasma lipolysis, which leads to a temporary decrease in triglyceride clearance due to inhibition of lipoprotein lipase.

Insulin may also affect lipase activity, but there is no evidence of an adverse effect of this factor on the therapeutic value of the drug.

Symptoms: Intralipid elimination disorders can lead to the development of excessive fat syndrome, which in turn may lead to renal dysfunction or infection. The syndrome of excessive fat load is characterized by hyperlipidemia, fever, fatty infiltration, impaired function of various organs and coma.

Treatment: Termination of Intralipid infusion leads to the disappearance of all symptoms.
