Prevention and treatment of fungal skin diseases:

  • trichophytosis, microsporia, rubrophytosis caused by sensitive pathogens, incl. mycoses of the feet and inguinal epidermophytosis;

  • yeast infection (skin candidiasis);

  • versicolor versicolor.

Outwardly. Before applying the drug Exifin, it is necessary to clean and dry the affected areas. The cream is applied in a thin layer to the affected skin and surrounding areas and rubbed lightly. For infections accompanied by diaper rash (under the mammary glands, in the interdigital spaces, between the buttocks, in the groin area), the places where the cream is applied can be covered with gauze, especially at night.

Average duration and frequency of application:

Dermatomycosis of the trunk, legs -1 week 1 time / day;

Epidermophytosis inguinal - 1-2 weeks 1 time per week.

Dermatomycosis of the feet - 1 week, 1 time per day. Skin candidiasis - 1-2 weeks 1 or 2 times / day.

Versicolor versicolor - 2 weeks 1 or 2 times / day.

The duration of the treatment depends on the indication and the severity of the course of the disease. A decrease in the severity of clinical manifestations is noted in the first days of treatment. 'For the prevention of recurrence of mycoses of the feet - 2-3 courses for 2 weeks with a break of 7 days. The dosage regimen of Exifin in the elderly does not differ from that described above.

Excipients: cetyl alcohol - 40 mg, stearyl alcohol - 40 mg, cetyl palmitate - 20 mg, benzyl alcohol - 10 mg, sorbitan stearate - 19 mg, isopropyl myristate - 80 mg, sodium hydroxide - 2 mg, polysorbate 60 - 61 mg, water purified - 718 mg.

Hypersensitivity to the components of the drug.

Precautions: hepatic and / or renal failure; alcoholism; oppression of bone marrow hematopoiesis; tumors; metabolic diseases; occlusive vascular disease of the extremities, psoriasis, pregnancy, lactation period, children under 12 years of age (lack of sufficient clinical experience of use).

pharmachologic effect

Antifungal drug for topical use with a broad spectrum of antifungal activity. At low concentrations, terbinafine has fungicidal activity against dermatophytes (Trychophyton rubrum, Trychophyton mentagrophytes, Trychophyton verritcosum, Trychophyton violaceum, Trychophyton tonsurans, Microsporum canis, Epidermophyton floccosum), molds and certain dimorphic fungi (Pityspare) (or pityspare) Activity against yeast is fungicidal or fungistatic, depending on the type of fungus. Terbinafine, by inhibiting squalene epoxidase in the cell membrane of the fungus, specifically inhibits the early stage of sterol synthesis in the fungal cell, which leads to ergosterol deficiency, intracellular accumulation of squalene and death of the fungal cell. Squalene epoxidase is not associated with the cytochrome P450 system,therefore, terbinafine does not affect the metabolism of hormones and drugs, the metabolism of which is associated with the cytochrome P450 system.

Pharmacokinetics

When applied topically, absorption is less than 5%, it has a slight systemic effect.

Overdose

When applied externally, overdose is unlikely. Symptoms in case of accidental ingestion: headache, nausea, gastralgia, dizziness, increased urination, rash. Treatment: activated carbon, if necessary, symptomatic supportive therapy.
