mild to moderate dementia of the Alzheimer's type (Alzheimer's disease, including probable);

mild to moderate dementia in Parkinson's disease.

The initial dose is 1.5 mg 2 times a day. When using the drug in patients who are especially sensitive to the effects of cholinergic drugs, treatment should be started with the use of the drug at a dose of 1 mg 2 times a day.

Dose selection

The initial recommended dose is 1.5 mg (0.75 ml of solution) 2 times a day. If, after at least 2 weeks of treatment, this dose is well tolerated, it can be increased to 3 mg (1.5 ml of solution) 2 times a day. In case of good tolerance of the dose taken by the patient, its further increase is possible - up to 4.5 mg (2.25 ml of solution) 2 times a day and then up to 6 mg (3 ml of solution) 2 times a day - with a time interval of at least 2 weeks after each dose increase. Adverse events, namely nausea, vomiting, abdominal pain, loss of appetite or weight loss, observed during treatment, may decrease after missing 1 or more doses of the drug. If adverse events persist, the daily dose of the drug should be reduced to the previous dose well tolerated by the patient.

The maintenance dose is from 1.5 to 6 mg 2 times a day. In order to achieve the best therapeutic effect, the dose of the drug should be kept at the maximum well-tolerated level.

The maximum daily dose is 6 mg 2 times a day.

Resuming the drug after a break

If the interruption in taking the drug was several days or more, treatment should be resumed from the initial dose to reduce the risk of recurrence of adverse reactions (for example, severe vomiting). A gradual increase in dose is carried out in steps, as described above.

In patients with impaired renal or liver function, adjustment of the dosage regimen of the drug is not required.

Equal doses of ExelonЃ, taken as capsules or as an oral solution, are interchangeable.

Recommendations for the use of oral solution

The required amount of solution should be removed from the bottle using the dispenser supplied with it.

The solution can be taken directly from the dispenser.

in a blister 14 pcs.; in a cardboard box 1, 2, 4 or 8 blisters.

in a 50 ml dark glass bottle complete with a dispenser; in a cardboard box 1 set.

hypersensitivity to rivastigmine, other carbamate derivatives or other ingredients that make up the drug;

patients with severely impaired liver function (since the use has not been studied);

children (the use of the drug in children has not been studied, therefore, it is not recommended to prescribe).

Carefully:

sick sinus syndrome or conduction disorders - sinoatrial and AV blockade (like other cholinomimetic drugs);

obstruction of the urinary tract and convulsive syndrome (cholinergic stimulation can increase the secretion of hydrochloric acid in the stomach, lead to increased obstruction of the urinary tract and exacerbation of the convulsive syndrome);

history of bronchial asthma or obstructive airway disease (as well as other cholinomimetics);

simultaneous administration with other cholinomimetic drugs (taking into account the pharmacodynamic properties of the drug ExelonЃ).

During the dose selection period, as well as with the use of other cholinomimetics, adverse events were observed within a short period after increasing the dose. The severity of adverse events may decrease in response to a decrease in the dose of the drug. Otherwise, the drug ExelonЃ should be canceled (see 'Side effects').
