  • Bacterial, viral, fungal eye diseases,

  • corneal diseases associated with epithelial defects, trachoma, glaucoma.

Pharmacodynamics

Dexamethasone is a synthetic fluorinated glucocorticosteroid that has no mineralocorticoid activity. It has a pronounced anti-inflammatory, anti-allergic and desensitizing effect. Dexamethasone actively suppresses inflammatory processes, inhibiting the release of inflammatory mediators by eosinophils, migration of mast cells and reducing capillary permeability, vasodilation.

Pharmokinetics

When applied topically, it penetrates well into the epithelium of the cornea and the conjunctiva: therapeutic concentrations are achieved in the aqueous humor of the eye; with inflammation or damage to the mucous membrane, the rate of penetration increases.

Overdose

In case of contact with the eyes of an excessive amount of the drug, it is recommended to rinse the eyes with warm water. No toxic effects are expected in case of overdose with topical application or if the contents of one vial are accidentally swallowed. Overdose symptoms may be local manifestations.

Treatment for overdose is symptomatic.

Side effects

The following adverse reactions have been reported in clinical trials:

Nervous system disorders: infrequently: dysgeusia.

Disturbances from the organ of vision: often: discomfort in the eyes. Uncommon: keratitis, conjunctivitis, 'dry' keratoconjunctivitis, corneal staining, photophobia, blurred vision, itching in the eye, foreign body sensation in the eyes, increased tearing, unusual sensations in the eye, crusting at the edges of the eyelids, eye irritation, redness of the eyes.

Adverse reactions, the frequency of which has not been established (post-marketing experience):

Immune system disorders: hypersensitivity.

Nervous system disorders: dizziness, headache.

Violations of the organ of vision: increased intraocular pressure, decreased visual acuity, corneal erosion, eyelid ptosis, eye pain, mydriasis.

It has been reported about very rare cases of the development of corneal calcification with previous significant damage due to the presence of phosphates in the preparation.

Drug interactions

Concomitant use of topical steroids and topical non-steroidal anti-inflammatory drugs may increase the likelihood of impaired corneal healing.

The risk of an additional increase in intraocular pressure cannot be excluded if dexamethasone is used in conjunction with anticholinergics, which can also cause an increase in intraocular pressure in predisposed patients. In the case of combined use with antiglaucoma drugs, it is possible to reduce the hypotensive effect of the latter.

In the case of use with other topical ophthalmic drugs, the interval between their use should be at least 10 minutes.

Eye ointments should be used last.
