Pharmacological action
Ciprofloxacin is a broad-spectrum antimicrobial drug from the group of fluoroquinolones. It is bactericidal. The drug inhibits the bacterial DNA gyrase enzyme, as a result of which DNA replication and the synthesis of bacterial cellular proteins are disrupted. Ciprofloxacin acts on both multiplying microorganisms and those in the dormant phase.
Sensitive to ciprofloxacin:
Gram-negative aerobic bacteria: Escherichia coli, Salmonella spp, Shigella spp, Citrobacter spp, Klebsiella spp, Enterobacter spp, Proteus mirabilis, Proteus vulgaris, Serratia marcescens sppa rfi, Haffnia, Morganella mordarii, Vibrio spp, Yersinia spp.
Other gram-negative bacteria: Haemophilus spp, Pseudomonas aeruginosa, Moraxella catarrhalis, Aeromona, spp, Pasteurella multocida, Plesiomonas shigelloides, Campylobacter jejuni, Neissenia spp.
Some intracellular pathogens: Legionella pneumophila, Brucella spp, Chlamydia trachomatis, Listeria monocytogenes, Mycobacterium tuberculosis, Mycobacterium kansasii, Mycobacterium aviumintracellulare.
Gram-positive aerobic bacteria: Staphylococcus spp. (S.haemolyticus, S. hominis, S. saprophyticus), Streptococcus spp. (St. pyogenes, St. agalactiae).
The sensitivity of bacteria Streptococcus pneumoniae, Enterococcus faecalis is moderate. Most methicillin-resistant staphylococci are also resistant to ciprofloxacin.

Indications
In ophthalmology: - Infectious and inflammatory eye diseases.
- Acute and subacute conjunctivitis.
- Blepharitis.
- Blepharoconjunctivitis.
- Keratitis.
- Keratoconjunctivitis.
- Bacterial corneal ulcer.
- Chronic dacryocystitis.
- Maybomit (barley).
- Infectious eye damage after injuries or foreign bodies.
- Pre- and postoperative prophylaxis of infectious complications in ophthalmic surgery.
In otorhinolaryngology: - Otitis externa.
- Treatment of postoperative infectious complications.
Contraindications
- Hypersensitivity.
- Viral keratitis.
- Children's age (up to 1 year - for eye drops).
Use during pregnancy and lactation
During pregnancy and lactation, ciprofloxacin should be used only when the possible benefits to the mother justify the potential risk to the fetus (baby).
Special instructions
The solution in the form of eye drops is not intended for intraocular injection.
When using other ophthalmic medicines, the interval between their administration should be at least 5 minutes.
Use of the drug should be discontinued if any signs of hypersensitivity appear.
The patient should be informed that if, after applying the drops for a long time, conjunctival hyperemia continues or accrues, then stop using the drug and consult a doctor.
Wearing soft contact lenses is not recommended during treatment with the drug.
When using hard contact lenses, they should be removed before instillation and put on again 15-20 minutes after instillation of the drug.
Composition of
Solution contains:
Active ingredient:
ciprofloxacin hydrochloride 3 mg per mg.
Excipients:
sodium acetate trihydrate,
mannitol,
disodium edetate dihydrate,
benzalkonium chloride,
acetic glacial acid,
purified water.
Dosage and Administration
Topically.
In ophthalmology: in case of mild to moderate infection, eye drops are instilled 1-2 drops in the lower conjunctival sac of the affected eye (or both eyes) every 4 hours, in severe infections - 2 drops every hour. After improvement, the dose and frequency of instillation is reduced.
In case of bacterial corneal ulcer - 1 drop every 15 minutes for 6 hours, then 1 drop every 30 minutes during waking hours for 2 days - 1 drop every hour during waking hours from 3 to 14 days - 1 drop every 4 hours during waking hours.
In otorhinolaryngology: the external auditory canal is carefully cleaned. It is recommended that when consumed, the solution has room temperature or body temperature in order to avoid vestibular stimulation.
The drug is instilled into the external auditory meatus. The recommended dose is 3-4 drops 2-4 times a day or more often, depending on the need. The patient should lie on the opposite side of the affected ear for 5-10 minutes after instillation.
In rare cases, after local cleansing, a cotton swab moistened with a ciprofloxacin solution can be placed in the ear in the external auditory canal until the next procedure.
In general, the duration of treatment should not exceed 5-10 days. In some cases, treatment may be extended if the local flora is sensitive.
Side effects
Allergic reactions: itching, burning, mild soreness and hyperemia of the conjunctiva or in the area of the external auditory canal and eardrum, nausea
rarely - swelling of the eyelids, photophobia, lacrimation, sensation of a foreign body in the eyes, an unpleasant aftertaste in the mouth immediately after instillation, a decrease in visual acuity, the appearance of a white crystalline precipitate in patients with a corneal ulcer, keratitis, keratopathy, the appearance of corneal spots or corneal infiltration, the development of superinfection.
Drug interaction
Interaction of drops of ciprofloxacin with other drugs with simultaneous use was not detected.
There is evidence that the systemic use of certain quinolones leads to an increase in theophylline concentration in blood plasma, affects the metabolism of caffeine and enhances the action of oral anticoagulants such as warfarin and its derivatives. a temporary increase in serum creatinine was reported in patients who were prescribed cyclosporine along with systemic use of ciprofloxacin.
Ciprofloxacin is not compatible with alkaline solutions.
Overdose
There is no overdose of ciprofloxacin when used as eye drops.
In the case of accidental administration of the drug inside the appearance of overdose symptoms is unlikely, since the content of the drug in one vial of drops is extremely small (15 mg), with a maximum daily dose for an adult of 1 g (500 mg for children).
