Pharmacological action

Antimicrobial derivative of fluoroquinolone. It is bactericidal. It inhibits bacterial DNA gyrase (topoisomerases II and IV, responsible for the process of supercoiling of chromosomal DNA around nuclear RNA, which is necessary for reading genetic information), disrupts DNA synthesis, bacterial growth and division, causes pronounced morphological changes (including cell wall and membranes) and the rapid death of a bacterial cell.

It acts bactericidal on gram-negative microorganisms during dormancy and division (since it affects not only DNA gyrase, but also causes lysis of the cell wall), on gram-positive microorganisms - only during the period of division.

Low toxicity to macroorganism cells is explained by the absence of DNA gyrase in them. While taking ciprofloxacin, there is no parallel development of resistance to other antibiotics that do not belong to the group of gyrase inhibitors, which makes the drug highly effective against bacteria that are resistant, for example, to aminoglycosides, penicillins, cephalosporins, tetracyclines and many other antibiotics.

Ciprofloxacin is active against gram-negative aerobic bacteria: Escherichia coli, Salmonella spp., Shigella spp., Citrobacter spp., Klebsiella spp., Enterobacter spp., Proteus mirabilis, Proteus vulgaris, Serratia marcescens, Edwards sardiusella , Morganella morganii, Vibrio spp., Yersinia spp., Haemophilus spp., Pseudomonas aeruginosa, Moraxella catarrhalis, Aeromonas spp., Pasteurella multocida, Plesiomonas shigelloides, Campylobacter jejuni, Neisseria spp. Gram-positive aerobic bacteria: Staphylococcus spp. (Staphylococcus aureus, Staphylococcus haemolyticus, Staphylococcus hominis, Staphylococcus saprophyticus), Streptococcus pyogenes, Streptococcus agalactiae.

The drug is also active against certain intracellular pathogens (Legionella pneumophila, Brucella spp., Chlamydia trachomatis, Listeria monocytogenes, Mycobacterium tuberculosis, Mycobacterium kansasii, Corynebacterium diphtheriae).

Streptococcus pneumoniae, Enterococcus faecalis, Mycoplasma hominis, Gardnerella spp., moderately sensitive to the drug, Mycobacterium avium-intracellulare (high concentrations are required to suppress them).

Bacteroides fragilis, Pseudomonas cepacia, Pseudomonas maltophilia, Ureaplasma urealyticum, Clostridium difficile, Nocardia asteroides are resistant to the drug.

The drug is inactive against Treponema pallidum.

Most methicillin-resistant staphylococci are also resistant to ciprofloxacin. Resistance of sensitive pathogens develops extremely slowly, because on the one hand, after the action of ciprofloxacin, there are practically no persistent microorganisms, and on the other hand, bacterial cells have no enzymes that inactivate it.

Pharmacokinetics of

Composition

Dosage and administration of

For mild to moderately severe infections, eye drops are instilled in 1-2 drops in the lower conjunctival sac of the affected eye (or both eyes) every 4 hours, in severe infections - 2 drops every hour. After improvement, the dose and frequency of instillation is reduced.

The bottle must be closed after each use. Do not touch the tip of the pipette to the eye.

Side effects

Local reactions: possible - allergic reactions, itching, burning, mild soreness and hyperemia of the conjunctiva, nausea rarely - swelling of the eyelids, photophobia, lacrimation, sensation of a foreign body in the eyes, an unpleasant aftertaste in the mouth immediately after instillation, decreased acuity , the appearance of a white crystalline precipitate in patients with corneal ulcers, keratitis, keratopathy, the appearance of spots or corneal infiltration.

No interaction of ocular forms of ciprofloxacin with other agents has been reported.

Overdose
