pharmachologic effect

Pharmacodynamics

Bromhexine 4 Berlin-Chemie has mucolytic (secretolytic) and expectorant effects. The mucolytic effect is associated with depolymerization and liquefaction of mucoprotein and mucopolysaccharide fibers.

Pharmacokinetics

When taken orally, bromhexine is almost completely (99%) absorbed within 30 minutes. Cmax in the blood is reached approximately 1 hour after administration. Bioavailability is 80% due to the effect of the first pass through the liver. Bromhexine in plasma binds to proteins, crosses the blood-brain and placental barriers. In the liver, bromhexine undergoes demethylation and oxidation; some of the metabolites formed (ambroxol) remain active. The limiting T1 / 2 is 16 hours due to slow reverse diffusion from tissues. Excreted by the kidneys. In chronic renal failure, the release of metabolites of bromhexine is impaired. With repeated use, bromhexine can accumulate.

Application during pregnancy and lactation

The use of the drug during pregnancy is possible only if the intended benefit to the mother outweighs the possible risk to the fetus. The use of the drug during lactation is contraindicated.

Side effects

Possible: nausea, vomiting, dyspeptic symptoms, exacerbation of peptic ulcer disease. Rarely: allergic reactions develop (skin rash, rhinitis, edema), shortness of breath, fever and chills, anaphylactic shock, dizziness and headaches, increased serum transaminases. Patients with sorbitol / fructose intolerance under the influence of sorbitol contained in Bromhexin 4 Berlin-Chemie may also experience: nausea, vomiting and diarrhea, decreased blood sugar levels (accompanied by tremors, cold sweat, palpitations, feeling of fear), increased activity hepatic transaminases (extremely rare). If side effects occur, the drug should be discontinued and a doctor should be consulted.

Interaction

Bromhexine 4 Berlin-Chemie can be administered concurrently with other drugs used in the treatment of bronchopulmonary diseases. Bromhexin 4 Berlin-Chemie is not prescribed concurrently with antitussives (including those containing codeine), as they can make it difficult to cough up sputum liquefied with Bromhexine 4 Berlin-Chemie. Bromhexin 4 Berlin-Chemie promotes the penetration of antibiotics (erythromycin, cephalexin, oxytetracycline, ampicillin, amoxicillin) into the lung tissue.

Overdose

The following symptoms are possible: nausea, vomiting, and other gastrointestinal disturbances. Treatment: there is no specific antidote. In case of an overdose, it is necessary to induce vomiting, and then give the patient a liquid (milk or water). Gastric lavage is recommended within 1-2 hours after taking the drug.
