Pharmacological action
Antiviral for external use, a synthetic analogue of purine nucleoside. The thymidine kinase of virus-infected cells actively converts acyclovir through a series of sequential reactions into acyclovir mono-, di- and triphosphate. The latter interacts with viral DNA polymerase and integrates into DNA, which is synthesized for new viruses. Thus, defective viral DNA is formed, which leads to suppression of replication of new generations of viruses.
Acyclovir is active against Herpes simplex virus types 1 and 2, Varicella zoster virus, Epstein-Barr virus and cytomegalovirus.
Pharmacokinetics
When applied topically, systemic absorption is minimal.
Indications
Treatment of skin infections caused by Herpes simplex viruses types 1 and 2, including primary and recurrent genital herpes, as well as herpes localized on the lips.
Contraindications
Hypersensitivity to acyclovir and valaciclovir in children and adolescents under 18 years of age.
Precautions: Pregnancy, lactation (breastfeeding), dehydration, renal failure.
Use during pregnancy and lactation
Use of acyclovir during pregnancy is possible in cases where the intended benefits to the mother outweigh the potential risk to the fetus.
If necessary, use during lactation should decide on the termination of breastfeeding.
Special instructions
Herperax ointment should not be applied to the mucous membranes of the mouth, eyes, nose and vagina. The effectiveness of the treatment will be the higher the sooner it is started (at the first sign of infection).
To realize the therapeutic effect of acyclovir, the state of the body's immune system matters. Patients with reduced immunity (with HIV / AIDS or after bone marrow transplantation) against the background of topical use of acyclovir ointment should be prescribed systemic administration of the drug, as well as in cases of severe recurrent course of herpes infection.
Acyclovir does not prevent sexual transmission of herpes, therefore during treatment it is necessary to refrain from sexual intercourse, even in the absence of clinical manifestations.

Excipients: polyethylene glycol 4000, polyethylene glycol 1500, polyethylene glycol 400.
Dosage and Administration
Externally. Apply 4-6 times / day (every 4 hours) with a thin layer on the affected and adjacent skin areas. The drug is applied either with a cotton swab or with clean hands to avoid additional infection of the affected areas. Therapy should be continued until a crust forms on the vesicles, or until they completely heal. The duration of therapy is an average of 5 days and should not exceed 10 days.
It is important to begin treatment for a recurring infection during the prodromal phase or at the very onset of infection.
Duration of treatment - at least 5 days. In the absence of healing, treatment can be continued for up to 10 days. If symptoms persist for more than 10 days, you should consult your doctor.
Side effects
Determination of the frequency of adverse reactions: very often (? 1/10), often (? 1/100, <1/10), infrequently (? 1/1000, <1/100), rarely (? 1/10 000, <1/1000) and very rarely (<10 000), the frequency is unknown (the frequency of occurrence cannot be determined based on available data).

Symptoms (if swallowed): headache, neurological disorders, shortness of breath, nausea, vomiting, diarrhea, impaired kidney function, lethargy, convulsions, coma.
