Pharmacological action
Acyclovir is an antiviral drug, a synthetic analogue of the acyclic purine nucleoside, which has a highly selective effect on herpes viruses. Inside the virus-infected cells, a series of sequential reactions of the transformation of acyclovir into acyclovir mono-, di- and triphosphate undergoes a viral thymidine kinase.
Acyclovir triphosphate integrates into the viral DNA chain and blocks its synthesis through competitive inhibition of viral DNA polymerase. In vitro, acyclovir is active against Herpes simplex type 1 and 2, Varicella zoster higher concentrations are required to inhibit the Epstein-Barr virus. In vivo, acyclovir is therapeutically and prophylactically effective primarily for viral infections caused by Herpes simplex.
Indications
- treatment of infections of the skin and mucous membranes caused by Herpes simplex virus type 1 and 2, both primary and secondary, including genital herpes
- prevention of exacerbation of recurrent infections caused by Herpes simplex virus type 1 and 2 in patients with normal immune status
- prevention of primary and recurrent infections caused by Herpes simplex virus type 1 and 2 in patients with immunodeficiency
- as part of complex therapy in patients with severe immunodeficiency: with HIV infection (AIDS stage, early clinical detecting and detailed clinical picture) and in patients undergoing bone marrow transplantation
- treatment of primary and recurrent infections caused by the virus Varicella zoster (chickenpox, shingles).
Contraindications
Hypersensitivity to acyclovir, ganciclovir, valacyclovir or the components of the drug, lactation, children under 3 years of age.
Caution:
- pregnancy
- old age
- dehydration
- renal failure
- neurological disorders or neurological reactions to taking cytotoxic drugs (including history).
Use during pregnancy and lactation
Use during pregnancy is only possible if the intended benefit to the mother outweighs the potential risk to the fetus.
If you need to use the drug during lactation, you should stop breastfeeding.
Special instructions
Acyclovir should be prescribed with caution to patients with impaired renal function, elderly patients due to an increase in the half-life of acyclovir.
When using the drug, it is necessary to ensure the flow of a sufficient amount of fluid.
Ointment is not recommended to be applied to the mucous membranes of the mouth, eyes, genitals, because the development of severe local inflammation is possible.
The effectiveness of the treatment with the ointment will be the higher the sooner it is started.
In patients with immunodeficiency with repeated repeated courses of treatment, the formation of resistance of viruses to acyclovir sometimes occurs. When taking the drug, kidney function (blood urea and blood plasma creatinine) should be monitored.
Composition of
1 tab.:
- acyclovir 400 mg.
Excipients:
microcrystalline cellulose,
polyvinylpyrrolidone,
indigo carmine,
magnesium stearate,
sodium starch glycolate,
purified water.
Side effects of the digestive system: nausea, vomiting, diarrhea, abdominal pain rarely — a reversible increase in bilirubin and the activity of “liver” enzymes.
From the hemopoietic system: very rarely - anemia, leukopenia, thrombocytopenia.
From the urinary system: rarely - increased levels of urea and creatinine in the blood are very rare - acute renal failure.
From the side of the central nervous system: headache, weakness, dizziness, increased fatigue, confusion, hallucinations, drowsiness, paresthesias, convulsions, decreased concentration, agitation.
Allergic reactions: itching, rash, Lyell's syndrome, urticaria, erythema multiforme exudative, incl. Stevens-Johnson syndrome, angioedema, anaphylaxis.
Other: fever, lymphadenopathy, peripheral edema, visual impairment, myalgia, alopecia.
Drug Interactions
With simultaneous use with probenecid, the average elimination half-life increases and the clearance of acyclovir decreases.
With concomitant use with nephrotoxic drugs, the risk of developing renal dysfunction increases.
Storage Conditions
Store in a dry, dark place, out of the reach of children, at a temperature not exceeding 25 РC.
