Pharmacological action of

Pharmacokinetics of

Fluimucil is well absorbed by oral administration. It immediately deacetylates to cysteine ​​in the liver. In the blood, a mobile equilibrium of free and plasma protein-bound acetylcysteine ​​and its metabolites (cysteine, cystine, diacetylcysteine) is observed. Due to the high “first pass” effect through the liver, the bioavailability of acetylcysteine ​​is about 10%. Acetylcysteine ​​penetrates into the intercellular space, mainly distributed in the liver, kidneys, lungs, bronchial secretions.

The maximum plasma concentration is achieved 1-3 hours after oral administration and is 15 mmol / L, the connection with plasma proteins is 50%. T1 / 2 - about 1 hour, with cirrhosis, it increases to 8 hours. It is excreted by the kidneys in the form of inactive metabolites (inorganic sulfates, diacetylcysteine), a small part is excreted unchanged through the intestines. Penetrates through the placental barrier.

Pharmacodynamics

Mucolytic agent, dilutes sputum, increases its volume, facilitates the separation of sputum. The action is associated with the ability of the free sulfhydryl groups of acetylcysteine ​​to break the intra- and intermolecular disulfide bonds of acid mucopolysaccharides of sputum, which leads to the depolymerization of mucoproteins and a decrease in the viscosity of sputum. It remains active with purulent sputum.

Increases the secretion of less viscous sialomucins by goblet cells, reduces the adhesion of bacteria on the epithelial cells of the bronchial mucosa. Stimulates mucous cells of the bronchi, the secret of which lyses fibrin. It has a similar effect on the secret formed in inflammatory diseases of ENT organs. It has an antioxidant effect due to the presence of an SH group capable of neutralizing electrophilic oxidative toxins.

Acetylcysteine ​​easily penetrates into the cell, deacetylated to L-cysteine, from which intracellular glutathione is synthesized. Glutathione is a highly reactive tripeptide, antioxidant, cytoprotector that traps endogenous and exogenous free radicals and toxins. Acetylcysteine ​​prevents depletion and promotes an increase in the synthesis of intracellular glutathione, which is involved in the oxidation-reduction processes of cells, thereby contributing to the detoxification of harmful substances. This explains the effect of acetylcysteine ​​as an antidote for paracetamol poisoning.

Protects alpha1-antitrypsin (an elastase inhibitor) from the inactivating effects of HOCl, an oxidizing agent produced by myeloperoxidase active phagocytes. It also has an anti-inflammatory effect (due to the suppression of the formation of free radicals and active oxygen-containing substances responsible for the development of inflammation in the lung tissue).

Contraindications

Special instructions

For patients with bronchial asthma and obstructive bronchitis, acetylcysteine should be prescribed with caution because of the possibility of developing bronchospasm. In case of bronchospasm, treatment with acetylcysteine should be stopped immediately.

The use of acetylcysteine, mainly at the beginning of treatment, can cause thinning of the bronchial secretion and increase its volume. If the patient is unable to cough up sputum effectively, postural drainage or aspiration of sputum from the bronchi is necessary.

Fluimucil contains aspartame, a source of phenylalanine. This should be considered for patients suffering from phenylketonuria.

Also, the drug contains sorbitol and sucrose, which should not be used in patients with hereditary fructose intolerance, as well as malabsorption of glucose-galactose and sucrose-isomaltase deficiency.

It is recommended to take with caution in patients with a history of gastric and duodenal ulcers, especially in cases of concomitant use of other drugs that irritate the gastric mucosa.

Dosage and administration

For oral administration.

Adults: 5 ml of an oral solution of 40 mg / ml in 1-3 doses (corresponding to 200 - 600 mg of acetylcysteine per day).

Children:

- children from 2 to 5 years: 5 ml of oral solution 20 mg / ml in 2-3 doses (corresponding to 200-300 mg of acetylcysteine per day)

- children and adolescents from 6 to 14 years: 5 ml oral solution of 20 mg / ml in 3-4 doses (corresponds to 300-400 mg of acetylcysteine per day) or 5 ml of oral solution of 40 mg / ml in 2 doses (corresponds to 400 mg of acetylcysteine per day)

- children over 14 years: 5 ml of a solution for oral administration of 40 mg / ml in 1-3 doses (corresponding to 200 - 600 mg of acetylcysteine per day).

In case of paracetamol overdose: in the first 10 hours after administration, Fluimucil is administered as soon as possible at a rate of 140 mg / kg, longer at a rate of 70 mg / kg every 4 hours, for 1-3 days.

Drug Interactions

The combined use of acetylcysteine ​​with antitussive agents may increase sputum congestion due to suppression of the cough reflex.

When used simultaneously with antibiotics such as tetracyclines (excluding doxycycline), ampicillin, amphotericin B, their interaction with the thiol group of acetylcysteine ​​is possible, which leads to a decrease in the activity of both drugs.

Therefore, the interval between doses of these drugs should be at least 2 hours.

The simultaneous use of acetylcysteine ​​and nitroglycerin can lead to increased vasodilator and disaggregant effects of the latter. If simultaneous administration of acetylcysteine ​​and nitroglycerin is necessary, patients should control hypotension, which can be severe in nature and should be warned about the possibility of the appearance or development of headaches.

Activated carbon may decrease the effectiveness of acetylcysteine.

Acetylcysteine ​​eliminates the toxic effects of paracetamol.

Overdose of

Symptoms: An overdose can manifest with gastrointestinal symptoms such as nausea, vomiting, and diarrhea.

Treatment: symptomatic therapy. There is no specific antidote.
