Pharmacological action

Pharmacodynamics

Antifungal drug for topical use, is allylamine and has a wide spectrum of antifungal activity. It is characterized by pronounced epidermotropy. The target for terbinafine is the cytoplasmic membrane of the fungal cell.

Terbinafine acts in the early stages of sterol metabolism, at the level of the squalene epoxidase cycle. Suppressing the enzyme squalene epoxidase, terbinafine inhibits the formation of ergosterol - the main component of the cell wall of fungi, preventing their further reproduction. Squalene epoxidase is not associated with the P450 system.

The fungicidal effect is due to the accumulation of squalene in the fungal cell, which extract lipid components from the membranes. The lipid granules that accumulate inside the cell, gradually increasing in volume, break the defective cytoplasmic membranes, leading to the death of the fungal cell.

Terbinafine is highly active against all dermatophytes combined in the genera Trichophyton, Microsporum and Epidermophyton.

Terbinafine in small concentrations has a fungicidal effect against dermatophytes (Trichophyton rubrum, T.mentagrophytes, T.verrucosum, T.violaceum Microsporum canis and Epidermophyton floccosum) mold (Aspergillus spp., Scopulariopsis brevicaulisidae, Cand , C. tropicalis, C. parapsilosis, C. guillermondi, C. kursei, C. pseudotropicalis, Pityrosporum orbiculare (P. ovale and Malassezia furfur) dimorphic fungi (Sporothrix schenckii, etc.).

The effect of terbinafine on yeast-like fungi, depending on their species, can be fungicidal (Candida parapsilosis) or fungistatic (Candida albicans). Fungoterbin® also has anti-inflammatory, antipruritic, keratolytic effects. Due to the presence of urea in the cream, the drug has a moisturizing effect, promotes a deeper penetration of terbinafine into the skin.

Pharmacokinetics

When applied topically, the drug is absorbed very slightly (absorption - 5%) and accumulates in the skin, sebaceous glands, hair follicles, subcutaneous tissue and exerts its effect mainly at the site of application.

Indications

Fungal skin diseases caused by sensitive pathogens (trichophytosis, microsporia, epidermophytosis, rubrophytia, candidiasis of the skin):

mycosis of the foot / fungus of the foot (tinea pedis)

mycosis of the trunk (tinea mycosis mycosis of the skin mycosis corpus mycosis mycosis corpus large folds

inguinal epidermophytosis (tinea craris),

superficial candidiasis of the skin caused by fungi of the genus Candida (e.g. Candida albicans)

versicolor versicolor (pityriasis versicolor).

Contraindications

Hypersensitivity to terbinafine and / or other components of the drug.

Precautions

Severe renal or hepatic insufficiency, chronic alcoholism, inhibition of bone marrow hematopoiesis, tumors, metabolic diseases, occlusive vascular diseases of the extremities, pregnancy, lactation, children under 12 years of age (lack of sufficient experience of use).

Use during pregnancy and lactation

Experience with the drug during pregnancy is limited, so you should not use Fungoterbin during pregnancy. Terbinafine is excreted in breast milk, so if it is necessary to prescribe the drug, it is necessary to decide on the termination of breastfeeding.

Dosage and Administration

Externally.

Before use, clean and dry the affected areas. Fungoterbin is applied 1 or 2 times a day to affected areas of the skin and adjacent areas. The cream is applied in a thin layer and lightly rubbed. When applied to the area of ​​skin folds (inguinal, intergluteal, under the mammary glands), it is recommended to lay a gauze strip / bandage.

Dermatomycosis of the trunk, large folds, feet, legs: 1 time per day, treatment duration - 1 week.

Dermatomycosis of the feet: 1 time per day, duration of treatment - 1 week.

Candidiasis of the skin: 1 - 2 times a day, the duration of treatment - 1 - 2 weeks.

Multi-colored lichen: 1 - 2 times a day, treatment duration - 1 week.

Side effects

Local reactions: itching and burning of the skin, hyperemia (redness) at the site of application. Discontinuation of treatment is not required.

Allergic reactions: generalized rash, urticaria, angioedema. Treatment should be discontinued and consult a doctor.

Overdose

When used externally, it is unlikely.

Symptoms: headache, nausea, gastralgia, dizziness, increased urination, rash may occur with accidental ingestion of the cream.

Treatment: activated charcoal, if necessary - symptomatic and supportive therapy.
