Pharmacological action
Antifungal drug for external use. Terbinafine - the active substance of the drug Fungoterbin®, represents allylamine and has a wide spectrum of antifungal activity. Terbinafine disrupts the early stage of biosynthesis of the main component of the fungal cell membrane (ergosterol) by inhibiting the squalene epoxidase enzyme. Squalene epoxidase is not associated with the cytochrome P450 system. The fungicidal effect is due to the accumulation of squalene in the fungal cell, which extracts lipid components from the membranes. The lipid granules that accumulate inside the cell, gradually increasing in volume, break the defective cytoplasmic membranes, leading to the death of the fungal cell. Small concentrations of terbinafine have fungicidal activity against dermatophytes (Trichophyton rubrum, Trichophyton mentagrophytes, Trichophyton tonsurans, Trichophyton verrucosum, Trichophyton violaceum, Microsporum canis, Epidermophyton floccosum), yeast fungi (mainly Candida albicans) and some dimorphic fungi (Pityrosporum orbiculare). Activity against yeast-like fungi, depending on their type, can be fungicidal or fungistatic (prevents the further propagation of yeast-like fungi). Urea, which is part of the spray, has keratolytic (softening and causing rejection of the stratum corneum of the epidermis affected by a fungal infection) and moisturizing effects, urea is able to increase the dermatopenrance (penetration into the skin) of the dosage form and, thus, create high concentrations of terbinafine in the skin. Urea promotes rapid recovery of the epidermis. Butylhydroxytoluene is an antioxidant that stimulates regeneration processes. When applied topically, it inhibits free radical reactions.
Indications
Fungal skin diseases caused by dermatophytes such as Trichophyton (including T. rubrum, T. mentagrophytes, T. verrucosum, T. violaceum), Microsporum canis and Epidermophyton floccosum: foot mycosis / “foot fungus” (tine pedis) trunk mycosis (tinea corporis), including smooth skin mycosis and mycosis of large folds of the inguinal epidermophytosis (tinea cruris). Superficial skin candidiasis caused by fungi of the genus Candida (e.g. Candida albicans). Multi-colored lichen (Pityriasis versicolor), called by Pityrosporum orbiculare.
Contraindications
Hypersensitivity to terbinafine and / or other components of the drug, children's age up to 12 years (lack of sufficient experience of use). Precautions: Severe renal or hepatic insufficiency, chronic alcoholism, inhibition of bone marrow hematopoiesis, tumors, metabolic diseases, occlusive vascular diseases of the extremities, pregnancy, lactation.
Use during pregnancy and lactation
Experience with the drug during pregnancy is limited, so you should not use Fungoterbin during pregnancy. Terbinafine is excreted in breast milk, therefore, if it is necessary to prescribe the drug, it is necessary to decide on the termination of breastfeeding.
Composition
1 g of spray contains active substance: terbinafine hydrochloride - 10.0 mg excipients: urea - 10.0 mg, butylhydroxytoluene - 10.0 mg, ethanol 95% - 150.0 mg, low molecular weight povidone - 10.0 mg, propylene glycol - up to 1.0 g.
Dosage and Administration
Externally. The drug is applied to the skin once or twice a day. Before applying the spray, it is necessary to clean and dry the affected areas. The drug is sprayed onto the affected areas in an amount sufficient for their thorough moisturizing, and, in addition, they are applied to adjacent areas of both affected and intact skin. The average duration of treatment: Dermatomycosis of the trunk, large folds, legs, feet: 1 week 1 time per day. Candidiasis of the skin: 1-2 weeks 1 or 2 times a day. Multi-colored lichen: 1 week 1 or 2 times a day.
Side effects of
In rare cases, redness of the skin, a feeling of itching or burning are possible at the places of application of the spray, however, discontinuation of treatment due to these phenomena is rarely required. These symptoms should be distinguished from allergic reactions that occur rarely, however, when they occur, you must stop treatment and consult a doctor - generalized rash, urticaria, angioedema.
