Pharmacological action
Neo-Penotran is a combined preparation for intravaginal administration with antifungal, antiprotozoal and antibacterial effects.
Miconazole nitrate is active against pathogenic fungi, especially Candida albicans, as well as some gram-positive bacteria.
Metronidazole has an antiprotozoal and antibacterial effect.
Active against Trichomonas vaginalis, Gardnerella vaginalis, and also obligate anaerobic bacteria (including Peptostreptococcus anaerobius).
Indications
- vaginal candidiasis
- trichomonas vaginitis
- bacterial vaginosis
- mixed vaginal infection caused by pathogens of bacterial vaginosis, vaginal candidiasis and trichomonas vaginitis.
Contraindications
- I trimester of pregnancy
- lactation (breastfeeding)
- hypersensitivity to the components of the drug and their derivatives.
Use during pregnancy and lactation
Prescribing Neo-Penotran is contraindicated in the first trimester of pregnancy. In the II and III trimester of pregnancy, Neo-Penotran fote can be prescribed only according to strict indications and under the close supervision of a doctor.
If necessary, the appointment of Neo-Penotran during lactation, breastfeeding should be stopped, because metronidazole is excreted in breast milk. Breastfeeding can be resumed 24-48 hours after the end of treatment.
Special instructions
Caution should be given to patients with severely impaired liver function (including porphyria), peripheral and central nervous system diseases, and hematopoiesis disorders.
In case of vaginitis caused by Trichomonas vaginalis, simultaneous treatment of the sexual partner with metronidazole for oral administration is advisable.
When prescribing Neo-Penotran, patients should be warned that alcohol should be avoided during the period of use of the drug and within 24-48 hours after the end of the course of treatment because of the possibility of disulfiram-like reactions. With the simultaneous use of Neo-Penotran and a contraceptive diaphragm or condom, the suppository base can interact with rubber.
The drug is not recommended for virgins.
Suppositories should be used only intravaginally, they can not be swallowed or used in another way.
Use in pediatrics
The drug is not recommended for use in children.

Dosage and administration
Neo-Penotran forte is prescribed 1 vaginal suppository overnight for 7 days.
For recurrent vaginitis or vaginitis, resistant to previous therapy, the course of treatment should be increased to 14 days.
Vaginal suppositories should be inserted deep into the vagina using the disposable fingertips contained in the package.
When prescribing the drug to elderly patients (over 65 years old), a change in the dosage regimen is not required.
Side effects
Local reactions: burning, itching, irritation of the vaginal mucosa (2-6%).
From the digestive system: cramping abdominal pain (3%), metallic taste in the mouth (1.7%) in rare cases - dry mouth, constipation, diarrhea, loss of appetite, nausea, vomiting.
From the side of the central nervous system: headache, dizziness, ataxia, psycho-emotional lability, peripheral neuropathy (with an overdose and prolonged use of the drug), convulsions (less than 1%).
Dermatological reactions: skin rash (less than 1%).
From the hemopoietic system: leukopenia (less than 1%).
Other: changes in serum glucose (as determined by the hexokinase method).
interaction
With the simultaneous use of metronidazole with ethanol, disulfiram-like reactions occur.
With the simultaneous use of metronidazole with disulfiram, disorders of the central nervous system (mental reactions) can be observed.
With the simultaneous use of metronidazole enhances the effect of indirect anticoagulants.
With simultaneous use with lithium preparations, an increase in their toxicity may be observed.
With the simultaneous use of metronidazole with phenytoin, the level of phenytoin in the blood rises, and the level of metronidazole in the blood decreases.
Phenobarbital with simultaneous use reduces the level of metronidazole in the blood.
Concomitant use with cimetidine may increase blood levels of metronidazole.
Metronidazole and miconazole inhibit the metabolism of astemizole and terfenadine, as a result, the concentration of astemizole and terfenadine in the blood plasma increases.
It is possible to change the concentration of theophylline and procainamide in blood plasma with simultaneous use with Neo-Penotran.
Storage Conditions
